Did you know that you can view some of your records on the portal? Please see if what you are looking for is available to you.  If not, please 发送消息 with your request and provide the details of what information you are looking for. Your requested information will be sent to you via the portal. This is a quick and secure way to request your medical records!

OUWB Students: 4th year Elective Rotations

Submit your request through the portal by sending a message to a provider and attach your documents. Please specify what year you are in your program and the purpose of your forms.

Please allow up to 10 business days for the completion of up to 5 applications for 4th year elective rotations.

Applications in excess of five will be charged a $15.00 fee at the time of submission, per application.
